
You may be eligible for Head Start services if you meet the income guidelines and if:
-You have children from 3 through 5 years, or
-You have children with high risk factors (e.g. family receiving TANF or SSI), or
-Homeless / without stable housing, or
-You have children with special needs with an Individual Education Plan (IEP), or
-You have foster children, or
-You are a single parent working or attending school.
If you are eligible for one of these programs (per submission of interest form) you will be contacted to provide the following:
-Birth certificate (State Copy)
-Immunization Record
-Proof of Residence (utility bill, lease, etc.)
-Verification of Income (check stub, W2, tax return, DHS statement, etc.)
-Guardianship or Custody Documents (if applicable)
-Health Appraisal (Physical within 30 days of the first day of school)
