About this PreSchool

Head Start Enrollment: Applications must be filled out with a Head Start staff member. Parents are welcome to visit the Head Start office, located at 5225 W. Vliet Street, (414) 777-7850, to complete an enrollment folder. Children cannot be enrolled in Head Start without completing all the necessary forms. Parents are welcome to visit the Head Start office at MPS Central Services and complete their registration.Families will need to provide the following documents in order to complete a Head Start enrollment folder: (tm)Proof of income (check stub, W-2 form, SSI or tax forms) (tm) Immunization records (tm)Certified Birth Certificate (tm) Proof of residence
