Please note that your application is incomplete without the following:

Birth certificate of your child required to process application.

For pregnant mothers:

• Proof of pregnancy with expected due date (doctor's note, family planning note, prenatal exam).
• Mother s name will go on Student Registration.
• Proof of income for the past year required to process application.
• 1040 tax form or one month's pay stubs for everyone in the household.
• Also include AFDC, SSI, child support, and unemployment compensation, if applicable.
• Guardianship document, if needed required to process application.

Proof of address:

• Rental lease
• Electric
• Water
• Land line phone bill.
