Please return this application with the documents listed below.

Please note that your application is incomplete without the following:

Birth certificate of your child required to process application.

For pregnant mothers:

Proof of pregnancy with expected due date (doctor's note, family planning note, prenatal exam).

Mother's name will go on Student Registration.

Proof of income for the past year required to process application.

1040 tax form or one month's pay stubs for everyone in the household.

Also include

AFDC

SSI

Child Support

Unemployment compensation, if applicable.

Guardianship document, if needed required to process application.

Proof of address:

• Electric
• Water
• Land line phone bill.
