Pre-K/Head Start Registration2011-2012 Registration InformationEligibility Requirements for 2011-2012Children three or four years old by September 1, 2011; andMust meet income eligibility guidelines and/or qualify for free or reduced National Nutrition Program; orLimited proficiency in English; orHomeless; orDisabilitiesApplication Requirements The following items must be presented to apply for pre-kindergarten:Birth CertificateImmunization RecordSocial Security CardFamily Income Verification (2 recent check stubs for each member employed)Proof of residence in TCISD current within 30 days (lease agreement, utility bills or receipts with address on it, food stamps, Medicaid verification) If living in someone else's home, the homeowner must provide 2 proofs of residency from the list above, and the appropriate notarized TCISD registration form.Driver's LicenseIf student lives with someone else other than parents please provide notarized or court documentation.Heal
