About this PreSchool

Please note:

• School District Residency and Registration is required for 4 year old UPK classes.
• Bring the completed application with all of the supporting documents listed below to the nearest Head Start location listed on page 2.
• Copies of your documents will be made and you will be given a receipt.
• We will not accept incomplete applications.

Documents to bring:

• Child V Original Birth Certificate & KLOG V
• Current Immunization Record &KLOG V
• Health Insurance and/or Medicaid Card (If child has coverage)
• Proof of Income (W-2 from last year, Public Assistance Budget Sheet, 4 consecutive weeks worth of pay stubs, Unemployment Documentation, Child Support Payments, SSI Grant Letter, Foster Care Grant, Tax Form 1040)
• Court Papers Verifying Guardianship (if applicable)
