Applications accepted Monday - Thursday, 7 a.m. - 6 p.m., at CAP's main office, 4606 S. Garnett Road, in the Exchange Center East Building, First Floor. Paperwork during enrollment must be completed by a legal guardian. To complete a child's application, parents or guardians must bring:Birth CertificateCurrent Immunization RecordsProof of AddressMost Recent Tax Return*Proof of Any Public AssistanceIncomplete applications will not be accepted. You can significantly increase the chance that your child will be enrolled by including a copy of your child's most recent well-child check up.
