About this PreSchool

Eligibility:

Early Head Start child must be under 3 years of age (as of October 15th)

Head Start child must be at least 3 years of age (as of October 15th)

Required documents to complete the application:

Child's birth certificate or crib cardChild's Social Security Card (if available)

Child's Immunization record

Proof of address (Must reside in Douglas County)

Proof of Income for 12 months (1040, W-2, Pay stubs for 12 months, TANF or SSI statements)

If applicable, please provide:

Documentation if your child has an identified disability (IEP or IFSP)
