To enroll your child into the program, pick up a health packet from one of our three sites. This packet will contain a physical form, lead addendum, shot record, dental form and emergency form. Have your child's physician and dentist fill out the appropriate forms. You will complete the emergency form yourself. Once these documents are completed, contact any one of our Family Services Coordinators to set up an intake appointment. On the day of your scheduled appointment you must bring: Initial Health Packet Child's Birth Certificate Child's Health Insurance Card Proof of Family Income Proof of Address There is No Cost for our program. However, your income must meet the federal poverty guidelines in order to qualify. A Family Services Coordinator will explain the guidelines during your intake interview.
