Applications are taken by the Family Resource Specialists in the program. To contact the appropriate personnel, call (423) 246-6180 to contact the Family Resource Specialist in your area. Please don't delay due to the fact that slots are limited. PLEASE HAVE THE FOLLOWING ITEMS READY FOR THE FAMILY RESOURCE SPECIALIST WHEN THE APPLICATION IS TAKEN:1. Birth Certificate of the child2. Shot Record of the child3. Proof of income of everyone in the household4. TennCare or other insurance card5. Social Security number of all members of the household
