How Do I Apply?To complete the application you must bring birth verification (birth certificate, hospital certificate, or medical card) and proof of income for the last calendar year or the previous 12 months (tax statement, paycheck stubs, and documentation or child support, alimony unemployment, cash assistance, SSI, etc.).
