To enroll your child, please contact a Family Advocate at a site near you to see if you are eligible to receive Head Start services and to set an appointment to complete the necessary enrollment paperwork.You will be asked to verify your income by providing one of the following documents: (tm) A copy of your most recent Individual Income Tax Form 1040, or (tm) W-2 Forms, or (tm) Three (3) consecutive pay stubs, or (tm) Monthly public assistance check, or (tm) Child Support checks, or (tm) Foster Care checks, or (tm) A copy of any other verification deemed necessary to determine eligibility (tm) A signed statement from an employer on company letterhead You will also be asked to fill out a Pre-Enrollment form with the help of a Family Advocate, if necessary.
