Welcome To Otoe-Missouria Head Start! Thank you, for your interest in the Otoe-Missouria Head Start Program. We are pleased to provide you with an application for enrollment in our Program. In order for us to determine your eligibility, we need to receive the following information as soon as possible. Completed Application Proof of child's age (Birth Certificate/Baptismal Record) Proof of Income (Pay stub, W2, Letter of Employment) Current Immunization Record CDIB Social Security Card Copy of All Insurance that child is under (Sooner Care) Well Child Exam from your child's Primary Care Provider (Physical form is attached) Also if your child has any food allergies please submit a Doctors letter stating the allergy) These documents must be submitted to us before your application can be evaluated.
