If you or someone you know is interested in the Head Start Program, please bring the following items:

• Birth Certificate
• Proof of Income
• Immunization (Blue Shot Record)
• Social Security Cards for all household members
• Physical or Medicaid Screening
• Child's Medicaid Number
• Your child must be 3 years of age.
