APPLICATION:

Either print the application English, print the application Spanish, fill in by hand, and mail to the address belowORSave the application English, Save the application Spanish to your desktop, type in the responses needed, print and mail.

Once they have received your application, you will be contacted and asked to come into the office to complete your enrollment paperwork.

At this time, eligibility for financial assistance will be calculated.
